Advance Care Planning

| FreeLearn the importance of advanced care options and have your questions answered by a representative of Alpha Home Health and Hospice.
Join a representative with Alpha Home Health and Hospice as they discuss the importance and benefits of doing advance directives and Portable Orders for Life-Sustaining Treatment (POLST) forms. The reasons for assigning a healthcare durable power of attorney will also be covered. There will be availability on a later date to meet one-on-one with the instructor to help complete the forms and answer additional questions.
This class is free and open to anyone in the community. Pre-registration is required.
